If you have been prescribed a physical therapy plan it is important to wear comfortable, loose clothing and tennis shoes.  Please consider wearing loose fitted clothing around the area specific to your injury.  Example: wear a tank top if you are being treated for a neck or shoulder injury.






What should I wear to my appointment?

If you are unable to make it to your appointment please kindly notify our office as soon as possible.  We recognize that emergencies do arise and plans often change.  When you call, we will help you reschedule.  With advanced notice we can better accommodate yours and others needs.  Should you cancel an appointment with less than 24 hours notice, you will be charged a $50 cancellation fee.

Be sure to bring your current health insurance card, prescription for physical therapy (if you have one), a payment method for any copay you may be responsible for, and your new patient paperwork. You will reduce your waiting time on your first visit if you completely fill out the required new patient paperwork.

Nope, you can seek the services of a chiropractor or physical therapist without a prescription from your physician. In some rare cases, your insurance may require a physician to write a prescription to be seen by a physical therapist. To find out what your insurance requires, just call the number on the back of your insurance card.
